    FEES, INSURANCE AND PAYMENT INFORMATION

    The following pertains to my fee:

• $125.00 per hour per individual 50 minute session
  

• $150.00 per hour for couples per 50 minute session
  

• Will accept check, credit cards, debit cards and cash
  

• I am a Blue Cross Blue Shield PPO Provider and a PHCS Provider . If you are not a member of Blue Cross PPO or PHCS, I can supply a reciept to you for possible reimbursement from your insurance company.
  

SLIDING SCALE AND CANCELLATION POLICY FOR ALL CLIENTS

Under special circumstances, I will accept a sliding fee scale which in large part will depend on      your overall finances. 

Also, please be advised that once you book an appointment,  you must give 24 hours advance        notice to cancel or you will be charged for the time scheduled.
